The NY State Office for People with Developmental Disabilities Direct Support Professional (DSP) Questionnaire Deadline is May 2.  Please help boost participation by sharing this survey link with your Direct Support Professionals!

The DSP questionnaire is anonymous and designed to be completed by DSPs who are employed by your organization. The survey is designed to understand the work experience of DSPs across all domains as it relates to workforce recruitment and retention of DSPs.

These five domains will be covered in the questionnaire:

1. Protection from Harm
2. Connection and Community
3. Opportunities for Growth
4. Mattering at Work
5. Work Live Harmony

As a thank you for completing the questionnaire, you will be asked if you want to participate in a raffle for $150 gift cards.  Any information collected during the raffle will not be passed back to the survey and will just be used for the raffle.
